If you are an uninvolved party, at the time of your request you may be asked to provide a Freedom of Information Act FOIA Request Form. To have it ready to mail or walk in with, you can download Form G-639 and the instructions from U.S. Homeland Security. Uninvolved parties may only be allowed a redacted copy (at the discretion of the Records Officer).

Alternatively, if your traffic accident took place on the Interstate, you will need to download and mail in this Request for Officer’s Report (SCDMV Form FR-50) from the South Carolina Department of Motor Vehicles. Fill out the form as completely as possible and sign. Send 2 copies along with a check or money order payable to SCDMV for $6.00, to the address below:
